Hanson Street Easton, MD 21601 Main Purpose of Job The Coordinator Special Programs, Health Services III is to provide coordination of community services to eligible individuals with Developmental Disabilities who reside in Talbot County.
The CCS determines eligibility for DDA Services, provides referrals for services, accesses day/residential/support services, facilitates the annual Person Centered Plan meetings, assists the individuals and their families to develop an annual individual plan, monitors individuals on the waiting list, and assists in transition planning.
These efforts work to maintain the individuals in the community and to promote individual choice and their overall quality of life.
This position is open to employees of Talbot County Health Department only.
MINIMUM
Qualifications Education:
Graduation from an accredited high school or possession of a high school equivalency certificate.
Experience:
Two years of professional experience in assisting in the coordination of a health-related program(s) or referring people to governmental and private resources.
Notes:
1.
Candidates may substitute 30 college credit hours from an accredited college or university on a year-for-year basis for the required experience.

Candidates may substitute U.
S.
Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health-related field of work on a year-for-year basis for the required experience and education.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
